ATTENTION ALL FUSD EMPLOYEES
Employee Health and Welfare Benefits Open Enrollment
(May 17-June 18, 2004)
The Fontana Unified School District is
offering the following health and welfare plans to all eligible employees:
Kaiser, Blue Shield, Delta Dental, Safeguard Dental, Medical Eye Services and
District-paid Prudential Life Insurance.
The District will continue to offer the
Kaiser plan with a $10 copay for office visits, $10 prescriptions, and $35
emergency room visits (if not admitted to the hospital).
We will continue to offer the Blue Shield HMO
plan with a $10 copay for office visits, $10 for name brand prescriptions, $5
for generic prescriptions, $10 for chiropractic visits, and $35 for emergency
room visits (if not admitted to the hospital).
The Blue Shield Added Advantage POS plan will
also be available again this year to full-time employees only, with an
additional charge of $110.10 monthly. Covered services may be utilized as
a HMO plan, Preferred Provider plan, or Non-Preferred Provider plan with
different levels of copays ($10/visit, $20/visit, 30% respectively).
Both medical carriers will have the same
eligibility requirements regarding dependent children, ages 19-25 years.
Dependents must be: Full-time students in an accredited college with 8
semester units, dependent upon the subscriber for support and be unmarried, or,
mentally or physically handicapped, indefinitely.
Open Enrollment will be from May 17
through June 18, 2004. If employees wish to make any benefits changes,
it is necessary that forms be completed by June 18, 2004, and returned to the
Benefits Office located in the District Office complex building from 7:30 a.m.
to 4:30 p.m. These changes will be effective July 1, 2004. If
you have no changes, it is not necessary to contact the Benefits Office.
