Enrollment Procedures for 2009-10 School Year
Mass enrollment for all FUSD Preschools is currently underway. We will begin
2009-10 Open Enrollment for students in Preschool through Grade 8 on Monday, May
11, 2009.
The FUSD Enrollment Center processes the enrollments for all students in
Preschool through Grade 8. The Enrollment Center is located at 9548 Citrus
Avenue in Fontana (across the street from the Fontana High School campus) and
their telephone number is (909) 357-5000, ext. 7300. Bilingual staff is
available for Spanish-speaking parents.
The process will be much quicker if you pick-up an enrollment packet from the
Enrollment Center and complete it before returning to the Center. For all PreK*-8
students, please bring the following documents with you when you come to the
Enrollment Center to enroll your child:

Valid picture identification of parents or guardians. |
| 2. |
Proof of residency is required in the form of a current (within the last 30
days) utility bill, such as electric, gas, water or trash, in the parents’ name
and with the address of the residence. If a family resides with another family,
the person whose name is on the receipt will need to accompany the family to the
Enrollment Center to sign the Verification of Residency form. A current utility
bill and valid picture identification will be required. |

Birth Certificate, passport, baptismal or hospital record of the student. |

Immunization Record (California yellow card) must be up-to-date.

entering Kindergarten and First Grade. If your child needs a physical or
immunizations, our Clinic’s hours are 8 am to 4 pm, Monday through Friday. |
*Because our Preschools are State-funded, additional documentation is required
for enrollment.

For the 2009-10 school year, the first day of school will be:
- Preschools – July 27, 2009
- Single-Track Elementary Schools & Middle Schools – July 29, 2009
- Canyon Crest & Palmetto Elementary Schools, Tracks A, B and C – July 7, 2009
- Canyon Crest & Palmetto Elementary Schools, Track D – August 3, 2009
- High Schools – August 10, 2009
