completed form to the FUSD Transportation Department at 9820 Citrus Avenue
(Fontana, CA 92335) or Fax it to (909) 357-5264.
Buss Pass Applications are also available at the Fontana Unified School
District's Enrollment Center, located at 9548 Citrus Avenue (Fontana, CA 92335)
between the hours of 7:30 am and 4:30 pm, Monday through Thursday.
You will be notified my mail or phone if you are eligible
for transportation. |
