The Fontana Unified School District Enrollment Center is happy to announce that registrations for Preschool 2011-12 are just around the corner.  If your child is turning 4 years of age on or before December 31, 2011, you can enroll your child in preschool for the 2011-12 school year.

We would like to take this opportunity to share with you the new way preschool enrollments will be conducted for the 2011-12 school year:

• Due to budget cuts, we will only be able to process a limited number of enrollments per day (approximately 45) - No Exceptions.
• This year we will have a schedule for enrollments.  Each school will be assigned a specific week.  No other schools will be seen but the ones scheduled for that specific week.
• Follow the schedule according to the site you wish your child to attend.
• The schedule will be posted at school sites, preschool classrooms, Enrollment Center, FUSD website, and given out at FUSD parent meetings.
• When the day is full, tickets will be handed out so you can come back the next day or another day within your week.
• First come first serve. If you are not able to be seen during your school’s week you will need to come back when Open Enrollment starts on June 1, 2011.
• You will need to have all required documents at the time of enrollment.

PRESCHOOL ENROLLMENT PACKETS WILL BE AVAILABLE BY MARCH 7, 2011 AT THE ENROLLMENT CENTER.

Required Documentation at the Time of Enrollment:

In order to provide you with the best service possible it’s important that you pay close attention to all the required documents and that you bring them at the time of enrollment. Missing some of the requirements will result in a delay and/or having to return to complete the enrollment on another day within your scheduled week or until June 1, 2011 during Open Enrollment.

• Proof of all family income.  One complete month of recent employment checks stubs, from month preceding enrollment.  If both parents are working we need pay stubs from both parents at the time of enrollment. (See enrollment packet)
• Recent proof of family address.  Current utility bills: gas, water, electricity, telephone or anything else that comes to you in the mail. (Rent receipts or agreements are not adequate for proof of address).
• Parent/Guardian picture I.D.
• Proof of age:  Birth certificate, hospital birth certificate, baptism record.
• Immunizations.  Need to be current for preschool, if not current, child will not be enrolled. (See enrollment packet)
• Physical examination.  This form needs to be completed by the physician (must be less than one year from child’s school start date).
• Child’s social security (optional) and medical insurance card (if applicable).
• Proof of family size.  To establish family size we need proof of other children in the home (under 18 years old) who are not enrolling in preschool.
  • Birth certificate, school records, or any other document indicating relationship of child to parent.
• Proof of head of household.  If only one parent is in the home bring rent receipt/utility bills.
• CUSTODY PAPERS.  If custody orders exist, please bring them at the time of enrollment.
• IEP.  If student is currently receiving LSH services, please bring copy of the IEP.
