Kindergarten enrollment for the 2004-05 school year will begin April 12, 2004 at District Enrollment Services (located at 9548 Citrus Avenue, Bldg. A in Fontana - across the street from Fontana High School).  Due to the large number of enrollments and in a continuing effort to improve service, District Enrollment Services will be splitting the schools into groups.  Below is the list of schools and the week we will be enrolling those students.

Enrollment packets can be picked-up at the school site or from District Enrollment Services starting March 22, 2004.  Please complete this paperwork before coming to District Enrollment Services to enroll your child.

Documents you will need to enroll your child:

• Parent/Guardian Picture I.D.
• Proof of Address:  current utility bill (within 30 days) - water, gas, electricity, trash, sewer or phone.  If you live with someone who pays the utilities, that person must come with you to do a Declaration of Residency (they must have a current utility bill in their name and picture I.D. with the current address).
• Child's birth certificate, passport, baptismal or hospital record
• Child's Immunization Records (every immunization must be dated, signed & stamped by the doctor or clinic)
• Child's Physical (must have doctor's signature, stamp, and health history date).  No appointments accepted.

Required Immunization

• Polio (OPV and/or IVP):  3 doses if one given on or after 4th birthday or 4 doses
• DTP (DTAP, DT/TD):  4 doses if one given on or after 4th birthday or 5 doses
• MMR:  2 doses
• Hepatitis B:  3 doses
• Varicella:  Required

Free immunizations are available for all FUSD students (2 months through 18 years old) at the FUSD Immunization Clinic.  No appointment necessary.

Physicals

Students are eligible for a free physical if they are:

• 0-18 years
• On regular MediCal
• Low/Moderate income
