Here are some of the top questions we have received regarding employee Benefits at Fontana Unified School District.

Q Does my Kaiser plan include chiropractic services?
 
A Although Kaiser does offer chiropractic services in their other health plans, our District plan does not.  However, Blue Shield plan does include this service.
 


Q When may I change my current plan or add a dependent that is not currently covered on my plans?
 
A November/December is our Open Enrollment period. Changes take effect in January.

Q At what age limit does my health plan cancel my dependents?
 
A Kaiser & Blue Shield: On the dependents 19th birthday, they must be enrolled as a full-time student and be at least 50% supported by the employee. Maximum age is 25.
Dental and Vision plans:  Maximum age is 25.  Dependents must meet either of the two requirements for health insurance.
 

Q If I ever leave my employment with the District, when will my benefits be canceled?
 
A At the end of the month in which you leave.
 

Q Is there something I can do to extend my benefits after I leave?
 
A Yes, there is COBRA!  A law that states all employees have the right to continue their insurance at their own cost.
 

Q What are my District paid benefit options for retirement?
 
A Certificated Employees need to be 55 years old and have worked in the District for 15 years,
Classified Employees need to be 50 years old and have worked in the district for 15 years,
Your choice will include 6 years of full benefits, which includes the health plan, dental plan, vision plan, and Life Insurance OR 8 years of your health plan only.

If you have worked 35 years or more, you are entitled to Life Time Medical!
