Home-Hospital Instruction

Home Hospital Instruction is a temporary placement, which provides one hour per day of home instruction by a credentialed teacher to students recovering from a non-communicable illness, surgery or injury. Temporary disability consists of out of school for 20 school days or more.

In order to begin the process, parent must obtain a Home Hospital Instruction Application and an Extension of Services Application from the student’s school site.

• Parent completes the Home Hospital Instruction Application.
• Parent submits the Extension of Services Application to their physician or psychiatrist to complete.
• Parent submits both forms, the completed Home Hospital Instruction Application and the Extension of Services Application (completed and signed by the physician or psychiatrist) to their child’s school site.
• Principal must sign the completed Home Hospital Instruction Application then forward both forms (the Home Hospital Instruction application and Extension of Services application) to the Elementary Instruction office.
• Student is placed on short-term independent study by the school site pending approval.
• The Elementary Instruction office will notify they parent upon approval.
