Central Records
The Central Records department provides school records to current and former students of the
Fontana Unified School District. The types of records that Central Records is able to provide include the following:
- Attendance Records
- Enrollment History Records
- Immunization Records
- Transcripts (official and unofficial)
Records may be requested by fax, mail or by coming into the
Central Records Office at 9680 Citrus Avenue, Fontana, CA 92335. Records requested by fax or mail take up to five (5) business days to process. Records requested in person
may be available immediately, although some requests may take up to five (5) business days to process.
When requesting records by
fax or mail, the following information must be included:
- A photo copy of a valid ID, such as a driver’s license or passport;
- Name the student was registered under when they attended
FUSD;
- Date of birth;
- Last school attended in FUSD, if known;
- Contact Phone Number;
- Signature and date signed;
- Type of record(s) requested; and
- Address records should be sent to
When requesting records
in person, please supply Student Records with the following information:

- Student ID Number, if known;
