K-8 Central Enrollment

All enrollments for grades PreK-8 are processed at the District Enrollment Center.

Location: 9548 Citrus Avenue in Fontana (across the street from Fontana High School)
Hours: Monday thru Friday, 8:00 a.m. to 5:00 p.m.
Enrollment Hours: Monday thru Friday, 8:00 a.m. to 4:45 p.m.

The following documents are required to register a student in the Fontana Unified School District. If you should need further information, please contact the Enrollment Center at (909) 357-5000 x7300.

1. Valid picture identification of parents/guardians
2. Proof of residency is required in the form of a current (within 30 days) utility bill (electric, gas, water, or trash) in the parents' names and with the address of the residence.
   If a family resides with another family, the person whose name is on the receipt will need to accompany the family to the Enrollment Center to sign the Verification of Residency.  A current utility bill and valid picture identification are required.
3. Birth Certificate, passport, baptismal or hospital record of the student.
4. Immunization Record, (CA yellow card) must be up-to-date. A Physical is required for students entering Kindergarten and First Grade.

The process will be quicker if you pick up an enrollment packet at the Center and complete it before returning to the Center.

If your child needs a physical or immunizations, the Clinic's hours are 8:00 a.m. to 4:00 p.m.
