ENROLLMENT PROCESS

STEP 1 – Gather your mandatory documents needed to complete the enrollment process at your appointment. These documents must be hard copies and present at time of your enrollment appointment.
- Parent/Guardian’s Picture ID
- Proof of Residency
- Proof of Child’s Birth Date
- Child’s Immunization Record
- Child’s Physical Form or Proof of Upcoming Physical Appointment (Students enrolling into TK, Kinder and 1st Grade Only) (Students entering FUSD from another country must have a completed physical and TB test).

STEP 2 – FOR PRESCHOOL ONLY – Please have the Preschool Enrollment Packet filled out prior to your appointment along with hard copies of all documents listed on page 3. You may pick this packet up at the Enrollment Center or District Office Building 16. You may also print out the Preschool Enrollment Packet by clicking on the link below.
STEP 3 – Appointments are held at the Enrollment Center 9548 Citrus Ave., Bldg. A., Fontana Ca 92335. We ask that you show up prepared with all mandatory documents and sign-in at our reception desk at least 10 minutes early. This will give you time to fill out some necessary forms before your appointment or you may print out the following forms, complete them and bring them with you to your appointment.

STEP 4 – FOR NEW FUSD STUDENTS (Students who have never been enrolled with FUSD): Click the orange “Pre-enroll” button below to begin the enrollment process. After you pre-enroll your student, you will then be able to make an appointment to complete the enrollment process.
FOR RETURNING FUSD STUDENTS (Students who have already attended a school in FUSD): You do not need to pre-enroll due to our system retaining your student’s basic information. Please, click the link below to automatically take you to our appointment system to schedule an appointment that works best for your schedule.
